Short and long text, single and multiple choice, rating, number, date and yes or no. Mark each question required or optional.
Choose who answers
Everyone or chosen sites, teams or roles. The form waits in their Needs you list until they answer.
Collect answers
Questions lock at the first answer, so every response answers the same form. A named answer can be accepted or handed back for a fix.
Read the results
For an anonymous form: totals per question only, never who said what and never a row-by-row export.
