The problem

Nobody remembers who changed the rule in March

When a balance is wrong or an hour is queried, the question is almost never what the record says now. It is what it said before, who changed it and when. The audit log answers that without anyone having to keep notes: every approval, rate change, policy edit, overtime decision and team change is written as it happens, to a record that is only ever appended to. Writing to it never blocks the action it describes, so it cannot become the reason something fails.
